- About the Company: Omega Sports is an American chain of sporting goods stores based out of Greensboro, North Carolina. The company was founded in 1978, and currently operates numerous locations throughout North Carolina.
How to Write
Step 1 – The first step is to download the document. Click the button/link near the top of this page to download the form as a .PDF file. The document can be completed in any .PDF editing program (or you can print it out and fill it in by hand).
Step 2 – Fill in the date in the upper right corner of the page (in the “Date” field). In the “Personal Information” subsection submit the following details:

- Are you at least 18 years or older?
- Are you eligible to work in the USA?
Step 3 – Employment Desired – In this subsection enter the following details into the appropriate input fields:
- Position
- Date you can start
- Pay rate desired
- Store location (select each location you want to apply to)
- Are you employed? If so, may we inquire with your current employer?
- Education information (name of school, location, dates attended, graduated?, etc.)
Step 4 – In the “Former Employers” subsection submit your employment history details (date/month/year, name of employer, title, pay, etc.).
Step 5 – Complete the “References” subsection (enter the names/contact details of your references).
