How to Write
Step 1 – The first step is to download the official application (press the download button at the top of this page).
Step 2 – Either print the application out or use a .PDF editing program to fill it in. On page one (1) of the form provide ALL of the following details:
- Last name
- First name
- Middle initial
- Street address
- City
- State
- Zip code
- Telephone number
- Mobile phone number
- Social security number
- Email address
- Referral source (check the appropriate box)
- Have you ever applied to a location before?
- Names of relatives/friends employed by the company
- Are you at least 18 years old?
- Are you legally employable in the United States?
- Do you speak a foreign language?
- Position desired
- Daily hourly availability information
- Preferred weekly hours
- Work history details (name of employer, position, reason for leaving, responsibilities, salary, etc.)
Step 3 – On the second page (2nd) of the form fill in the following details:
- Work history (continued)
- Explain any gaps in employment
- Explain any transportation limitations
- If hired can you provide proof of your citizenship/legal right to work in the country?
- Education information (schools, skills, training, etc.)
- References (names, telephone numbers, years known)
- AND
- Initial/sign/date the document
