- About the Company: Bahama Breeze is a franchise that serves frozen ice and related food products/beverages. The company is based out of Lubbock, Texas and currently has over 95 locations throughout the following states (Arizona, Alabama, California, Nevada, Utah, New Mexico, Texas, Florida, Georgia, Missouri, and Oklahoma).

How to Write
Step 1 – The first step is to download the document (click the button/link at the top of this page).
Step 2 – After downloading the form, you can either print it out or fill it in by using a .PDF editing program.
Step 3 – On page one of the form enter the following information into the appropriate input fields:
- Full name
- Current date
- Full street address
- Email address
- Social security number
- Work phone number
- Alternate phone number
- Positions applying for
- Answers to the required questions
- Start date
- Expected hourly rate
- Expected weekly earnings
- Work schedule availability
- Education history (name of school, dates attended, etc.)
- Work history (company name, job title, manager, location, etc.)
Step 4 – Read the disclosures and then submit your initials next to each disclosure. Sign your name, and then fill in the date (dd/mm/yyyy format).
