Step 10 – Contact Information (continued):
- Home phone number
- Work phone number
- Other phone (i.e. mobile) number

- Web address
Step 11 – Education History – Submit the following details regarding your education history:
- School or education information
- Major or area of study
- Degree
- GPA
- Grad year
Step 12 – Continue to submit the required information for the remaining sections/subsections of the form. After all of the required details have been entered into the form you will be able to submit the application for further processing.
